Please be advised that USCIS requires that you complete and sign a separate privacy release form for each petition that you filed. The petitioner is the only person who may request information on a pending petition. Electronic signatures are not accepted- you must print the form and sign it by hand. When you return the signed release, please make sure that you also submit a copy of your I-797 Notice of Action.

The privacy release is a legal document- please do not alter it. If you need more space, you may add a second page, but just make sure you sign and date it.

In the explanation of the problem on the privacy release (NOT in your email), please be sure to include all the following: (Please don’t just jot down the responses to the four items below, write an explanation of the problem and include the information requested.)

  1. Whether it is a first-time passport or a renewal
  2. Date of trip
  3. Country of destination
  4. Reason for the trip

IMPORTANT: On the first page, where it asks for your place of birth, please be sure to include both the City and the State (or the City and the Country). On the second page of the Privacy Act Waiver (the third page of the attachment), please make sure to indicate that you authorize the release of information to your individual Member of Congress and Staff by marking the YES box. It is recommended that you mark all the other boxes, “NO”.

You must also submit a copy of your travel itinerary.

ONLY IF APPLICABLE IF you have already submitted your application, you will also need to include on the privacy release the information below:

  1. Whether you paid the expedite fee and/or overnight delivery fees
  2. Application number
  3. Whether your check has been cashed

*If you are completing the privacy release for a child, be advised that the D.O.B. refers to the passport applicant (your child). Also, if the child is younger than 18, the parent should sign the release, but should indicate in parenthesis that the parent is the one signing on behalf of the child. For example: Parental signature followed by explanation in parenthesis: (Parent of XXX)

While waiting for an appointment, you should start preparing by getting your child’s photos and filling out the application form, DS-11. If both parents cannot accompany the child to the appointment, then the parent not attending should also complete DS-3053 to authorize the other parent to obtain a passport for the child. Be advised that form DS-3053 must be notarized. Both forms can be downloaded from the State Department’s website:https://travel.state.gov/content/travel/en/passports/need-passport/under-16.html

The expedite fee is $60.00 and the overnight delivery fee is $19.53.

Although we will work diligently to assist you, we cannot guarantee that your passport will be processed prior to your scheduled travel date. Furthermore, we cannot ensure that your passport will be issued the day of your appointment.

Also, please be advised that, if you do not reside in the Congresswoman’s district, we will refer you to the Member of Congress who represents you.

Be advised that our office does not decide when (or if) you will be granted an appointment and the Passport Agency rarely grants next-day appointments. However, in general, appointments are usually granted between two to five days prior to your date of travel. If you know ahead of time that there is a specific date that you definitely cannot attend an appointment at the Miami Passport Agency you must let us know by indicating this on the privacy release form. You must provide a justifiable reason for not being available (i.e. religious reason, attending a funeral, etc.). NOTE: On occasion, the Miami Passport Agency grants Saturday appointments. If you cannot attend a Saturday appointment, please make sure you state so in your explanation of the problem.
