You must fill out the entire form. If you have previously communicated with our office about a submissions request via email, phone, or meeting, you must still fill out the form and follow the directions below. Please do not hesitate to reach out if you have questions.
- All submissions must be emailed to email@example.com. The subject line of the email submission should look like the following: “FY 21 [Bill Name] Submission”, e.g. “FY 21 MilCon Submission”.
- The deadline to submit requests to our office is March 2, 2020. If this deadline presents a problem, please reach out as soon as possible, and we will work with you. If you have any questions, please reach out to the appropriate staffer who handles the subcommittee that your request involves.
- In addition to the attached form, please include a letter on your organization’s letterhead outlining the request.
- If you have more than one request, please use one form per request.
